FLORENCE SCHOOL DISTRICT ONE

STUDENT SAFETY CONTRACT

 

 

In order to ensure that science experiments are safe and positive learning experiences, students and their parents should read, discuss and sign this science safety contract.

 

SAFETY RULES:

 

 1)        Follow all instructions given by your instructor.  When uncertain about a procedure or directions, ask the instructor for clarification.  Do not attempt to use any equipment or materials until instructed.

 2)        Always wear ANSI A.87 safety goggles in lab.

 3)        If contact lenses are worn during labs involving chemicals, non-vented, labeled goggles must be worn.  All other appropriate personal protective equipment for the lab must be worn.

 4)        A serious attitude is needed at all times during lab activities.  No horseplay will be tolerated.

 5)        NO open-toed shoes are to be worn in the lab.  Long pants or long skirts should be worn in the lab.

 6)        NO gum, candy, drinks or other edible materials are allowed in the lab.

 7)        NO tasting or smelling is allowed unless instructed by the teacher.

 8)        Hands should be kept away from the face and mouth.

 9)        NO mouth pipeting is allowed.

10)       Always pour acid into water, NEVER WATER INTO ACID.

11)       Broken glassware must be placed in a specified container.

12)       Return all equipment and materials to designated areas and clean up thoroughly when finished with the lab activity.  Dispose of chemicals as instructed.

13)       Any accident must be properly reported to the instructor.

14)       Wash hands before leaving lab.

15)       Know the location and proper use of the following safety equipment:

            a)  fire extinguisher                                g)  Material Safety Data Sheets (MSDS)

            b)  fire blanket                                      h)  master shut off for gas and water

            c)  eye protective devices (goggles,       i)  heat sources (Bunsen burner, alcohol

                 face shields                                          lamps, microwave oven, hot plate,

d)  eyewash                                              heat lamps, etc.)

            e)  safety/drench shower                       j)  first aid kit

            f)  emergency call-back button 

16)       Know the proper safety procedures for the following situations:

            a)  fire                                                  d)  chemical spill

            b)  chemical splash to the body             e)  cuts

            c)  eye emergency                                 f)  burns

 

The student must pass a lab safety test with a minimum score of 80% before participating in lab activities

TO THE PARENT:

 

Your child will be working in the laboratory during this course.  In order to ensure his/her personal safety, it is important that the above rules are followed.  Failure to do so may result in your child’s being unable to participate in lab.  I understand these rules and agree that my child will abide by these and all other written and verbal instructions given in class.
